COVID-19 Regulations
AS A SPECTATOR:  I understand the following.....

1.  Masks must be worn at all times
2.  Social distancing will be followed
3.  The quarantine potential in the event someone near  you comes down with COVID-19.  The decision was made to hold Powder Puff on Sunday, May 16th so the seniors have a good 2 week window before graduation, in the event someone comes down with COVID-19 and needs to quarantine.  It is possible for all spectators to quarantine if someone does test positive for COVID-19.  We would hate for anyone to miss any end of year activities or school due to coming down with COVID-19 or needing to quarantine.
4.  I will assess the following before attending the Powder Puff Football Game.  If you reply YES to any of the questions in the checklist, please stay home and do not come to the Powder Puff game.  Do you have any ONE of the following symptoms?
*  Fever (temperature 100.4°F or higher) without having taken any fever reducing medications
*  Difficulty or trouble breathing
*  New cough or a cough that gets worse
*  New loss of taste or smell
Do you have at least TWO of the following symptoms?
*  Sore throat
*  Nausea
*  Vomiting
*  Diarrhea
*  Chills
*  Muscle pain
*  Excessive fatigue (extreme tiredness)
*  New or severe headache
*  New nasal congestion or runny nose
Have you, or anyone you have been in close contact with in the last 14 days, been diagnosed with COVID-19?
Have you been asked to self-isolate or quarantine by a medical professional or a local public health official?
Are you, or is any member of your household who is symptomatic (has symptoms consistent with COVID-19), currently waiting for COVID-19 test results?

If you have symptoms of COVID-19, stay home until all three of these things are true:
1.  You feel better. Your cough, shortness of breath, or other symptoms are better AND
2.  It has been 10 days since you first felt sick OR you have received a negative COVID-19 test result or alternative diagnosis AND
3.  You have had no fever for at least 24 hours without using medicine that lowers fevers.

By signing, I agree to comply with the “COVID-19 Regulations" listed above.   *
