State Assessment Parent/Guardian Opt-Out Form
All Public Schools are required to administer specific state assessments each year. Although the Michigan Department of Education has requested a waiver for spring 2021 testing, it has not been approved at this time. Therefore, Livonia Public Schools is preparing to administer state assessments for all students in third through eleventh grades.

We realize that some families may have health concerns and may be uncomfortable with their child participating in these assessments. If you desire to opt your child out of any spring 2021 assessments, please indicate so on this form. There is not an option for remote administration for any tests listed on this form. All tests will be given in-person.
