Fertility Matters Canada (FMC) is launching our second annual Fertility Awards, and we'd love for you to be part of it by nominating someone who is doing amazing work for people with fertility challenges. The nominee could be someone who works at a clinic, someone who advocates for better government policies around fertility treatment, or even just a friend who helps you when you're feeling discouraged.
Those who are selected to receive a Fertility Award will receive:
An invitation to attend our live (virtual) awards ceremony where they will have an interview with Fertility Matters Canada Executive Director, Carolynn Dubé, which will be viewed by a nation-wide audience
Recognition from Fertility Matters Canada on all social media channels including Instagram, Facebook, LinkedIn, and TikTok as well as website and email marketing
An opportunity to do a recording on the Fertility in Focus podcast
A trophy as a token to remind themselves of their impact going forward
An exclusive invite to our party downtown Toronto during Canadian Fertility Awareness Week
An awards badge to display on social and/or your website

Please review the nomination categories and criteria before filling out the form. Individuals/organizations can be nominated in one category only; multiple nominations for one individual/organization in various award categories will not be considered. Nominations will be reviewed by FMC’s Nominations Committee and FMC reserves the right to deem applicants ineligible if any information is incomplete, false, or misleading.
Recipients will be contacted in the week of February 25th and will be invited to participate in our virtual awards ceremony and join us for our party in Toronto.
