MHS Student ID Card Replacement Request
Complete the form below to request a new student ID card!

The cost of a new student ID card, lanyard & cover is $10.00.
The cost of a new student lanyard & cover only is $5.00.

The fee will appear in your PaySchools account/Genesis Parent Portal. Genesis accounts will be locked until the replacement fee is paid. Please allow for up to two (2) school days after submitting this form for the fee to appear. 

New ID cards will not be printed or distributed until AFTER payment has been received! 
  • Please allow 1-2 school days after payment for students to receive the new ID card.
  • Students will receive an email to their MTSD account when the replacement ID is ready for pickup.
