How this works: 
  • Rank the designs in order of your preference — 1st Choice being your most desired design, and the Last Choice being the one you want the least.
  • You may submit multiple forms for different order numbers to secure the designs you prefer for the earlier orders first.
  • If a prize does not have a design option to choose from, it is packed in blind packaging — the design will be a surprise.
Terms & Conditions:
  • Prizes will be assigned on a first-come, first-served basis (based on the submission time of this form).
  • Selecting and requesting a design does not guarantee you will receive it, as others may have chosen it before you and it may run out before your turn (or due to other unforeseen circumstances, the design may not be available)— but we will try our best to fulfil your request.
  • Please skip the questions for any items you did not win.
  • If there are too many designs and you don’t wish to rank all of them, feel free to leave the rest blank. Just rank the ones you really want — any unranked designs may be assigned at random if needed.
  • Any conflicting answers will be interpreted at our discretion when packing.
