This form is for requesting use of Cornerstone Congregational Church facilities for meetings and/or functions.  Please fill out this form well in advance of your meeting/function to obtain approval.

1.  A Certificate of Liability from vendors or contractors must be submitted along with this form.                                                             2.  For non-Cornerstone events a $50 deposit will be required and held for additional cleaning if needed. It will be returned if cleaning is satisfactory.
