New Mexico Summer Reading Program Incident Report Form for Community Questions and Incident Reporting
This form is to be used by constituents, participants, staff and community members for the New Mexico Summer Reading Program to report concerns, commendations and any other incidents that may occur on the Summer Reading sites.  This form will be monitored by staff for response.  Please note that for the Summer Reading Program, we will follow all IPRA and FERPA Guidelines.  Please see below.
