Confirmation Prep Registration Form
Confirmation Prep Classes will take place on Wednesday from 7-8:30 pm or on Sunday from 9:10 am- 10:20 am. All students must be entering their freshmen year of high school or already in high school.

If your student was not registered in catechesis or religious formation at the parish or enrolled in a Catholic high school where they participated in daily theology classes last year, please contact Abram Muenzberg, Dir. of Youth and Young Adult Ministries, at 505-771-8331 or abram@ccincarnation.org.

The Archdiocesan Sacramental Policy states:

12 Confirmation preparation is one of many opportunities for ongoing faith formation in the life of young people. It is the obligation of parents to see that their children continue to participate in the catechesis and other faith formation offered by the parish throughout their high school years, even after the celebration of confirmation. Confirmation preparation will include the following:
12.1 Prior to entrance into the final preparation period for confirmation, young people should have recently
completed at least one year of catechesis or religious formation at the parish or be enrolled in a Catholic high
school where they participate in daily theology classes.
12.2 A final period of four to six months of confirmation preparation is held at the local parish for all candidates,
including those attending Catholic high schools. This period of preparation is required and is to include
orientation, catechesis on the sacrament of confirmation, spiritual and community outreach activities, retreats, and
the practical and liturgical arrangements for the celebration.

The Archdiocese of Santa Fe also requires a Baptismal Certificate that has been issued in the last six months from the parish of Baptism. Please contact the parish of Baptism and request a "Baptismal Certificate with full notations of Sacraments received and a strike through any lines of Sacraments not received."

The year prior to entrance into the final preparation period for confirmation, young people should have completed a full year of catechesis or religious formation at the parish or be enrolled in a Catholic high school where they participated in daily theology classes. Has your child fulfilled this requirement?
The Archdiocese of Santa Fe also requires that all high school students preparing for confirmation attend a retreat. Church of the Incarnation's Confirmation retreat is November 3-5, 2017. As of today, will there be a conflict for your child to attend?
Media Authorization/Photo Release-I hereby give permission for my child to be photographed or videotaped at Church of the Incarnation. I authorize the photo/video to be published on the parish website, bulletin or other parish and/or Archdiocesan publications.
Child/Youth Protection Program--The Archdioceses of Santa Fe mandates that we offer a class every year to all students, teaching them ways to protect themselves, be safe and communicate potential danger to a parent or responsible adult. This class will take place the second week of class.
