Motor Vehicle Use Agreement

The safety of University students, faculty, staff, and the public is a central concern of Miami University. The operations of Miami University necessarily involve some faculty, staff, students, and volunteers operating vehicles and other motorized equipment.

The information you are being asked to provide will be used by Miami University personnel to determine your eligibility to drive university owned or leased vehicles. You are not required to provide this information but if you choose not to do so, you will not be approved to drive vehicles on university business.

This completed form will be submitted to Risk Management.  

HireRight will send you a request for information via email in order to obtain a Motor Vehicle Record Report from the Department of Motor Vehicles for each state where you have held a driver’s license in the past three years.  

You will need to complete the HireRight request at your earliest convenience.  Be advised that vehicles may not be driven until you are notified by Risk Management that you have been placed on the approved driver list.
