Open Letter to Medical School Admissions Committees Regarding the MCAT

To Medical School Admissions Committees:
 
We are writing to express our concern about the status of the MCAT examination for students applying during the 2020-2021 application cycle. We recognize the unique challenges admissions committees are facing during this application cycle, but we want to communicate how this process has impacted premedical students. The purpose of this letter is to therefore strongly recommend that all medical schools consider making the MCAT optional this year, which Stanford School of Medicine has already elected to do.
 
There is no doubt careful deliberation occurring at medical schools across the nation, and we want to acknowledge that these are difficult administrative decisions. Much of the uncertainty around this application process has been unavoidable. However, the impact that these changes have had on students are numerous. Some premedical students have come together to chronicle the obstacles they have faced this year as well as their proposed next steps. You can access their document at www.tinyurl.com/AAMCSEA.
 
As the students discuss in their open letter, the initial rescheduling period for the MCAT was set to begin at 3:00am PST. Due to technical difficulties, students remained on the website until 2:30pm PST. Any students who may have had responsibilities during this period would have been relegated to choosing dates towards the end of the cycle (or traveling to remote locations). Students with disabilities were particularly affected by this rescheduling process; they could not sign up for an exam with accommodations through the online platform and had to engage in a lengthy process that essentially precluded them from their desired test dates.
 
The AAMC has tried to take steps towards addressing these issues by increasing test availability, but they have elected to forego virtual administration of the exam due to concerns about integrity. Although we understand that the AAMC is attempting to navigate uncharted territory, we believe that conducting in person MCAT examinations at this time is a problem for the following reasons:
 
Safety

1) With COVID-19 cases spiking around the US, an in-person MCAT examination is a potentially dangerous situation. Even with the best risk mitigation strategies, the risk of transmission is not eliminated (with students stating proper protocol is not being followed at testing sites). Students who have had to undergo multiple cancellations since March will therefore be put in a position where they have to decide between their health and their future. This is an untenable position for students to be in, and our institutions have a responsibility to ensure that we protect our students.

In addition to safety concerns for premedical students, there also exists the potential for the virus to spread within their families and social networks. As some applicants have expressed on social media, concerns over exposing their parent with cancer or sibling with asthma to COVID-19 has made it difficult for them to study effectively.

In response to these safety concerns, the LSAT is only offering virtually proctored exams this year. If the AAMC cannot conduct the MCAT virtually, these safety concerns are clearly enough to warrant making the exam optional.

Public Health Stewardship

1) With widespread misinformation campaigns, much of the public is looking towards medical institutions for leadership during this pandemic. Continuing with business as usual and potentially exposing students to COVID-19 is not just a safety concern for students. It does not properly communicate to the public how serious this pandemic is. Standardized exams are not an essential service, and although it will put an additional burden on admissions committees to differentiate students, taking the step of making the MCAT optional will further communicate how seriously risk mitigation strategies should be taken by the public.

2) Testing is still limited in many localities. The possibility of some of these MCAT examinations turning into super spreader incidents is one we have to take seriously. For instance, if a student contracts the virus (either via transportation towards/back from the exam or during the exam itself) and is asymptomatic, they may continue to interact with others (particularly in states with lax policies in place). We may therefore inadvertently contribute to spreading this disease, when our responsibility is to contribute to curtailing it.

The premedical students who created the document above heard from at least one student who was awaiting a COVID test result. That student had to decide between potentially spreading the virus or giving up the test slot she had worked so hard to get. As medical institutions, we simply cannot put students in these kinds of positions.

Equity

1) Students who are taking the MCAT this late, right before they apply to medical school, are likely to be students who were less informed about the medical school application process. It is likely that URM and first generation students will be disproportionately represented in the students who have had to deal with MCAT delays and cancellations.

The COVID-19 pandemic has already exacerbated so many of the inequities in our society, and this may lead to even greater impacts on URM students applying to medical school. Countless students have already decided not to apply to medical school this cycle because of the MCAT situation. As more cancellations occur, the MCAT will continue to exclude even more students.

Historically, URM students have performed worse on the MCAT (largely influenced by SES and structural racism). In this midst of a pandemic, with families strained economically, these inequities would be further enhanced.

2) Black premedical students specifically are facing the compounding ramifications of COVID-19 and anti-Black racism in the United States. The additional stress of regular scheduling delays, cancellations, and confusion not only disproportionately harms them, but will further exacerbate their well-documented underrepresentation in medicine.

Making the MCAT optional would enable these students to still apply while mitigating the impact that current circumstances may have on their futures.

3) COVID-19 morbidity and mortality is not equally spread amongst different states, and MCAT exams taken during this pandemic will therefore no longer be standardized. Students who are in particularly hard hit states will face more MCAT examination cancellations and be required to take on more risks to complete their MCAT in person.

These students should be given the opportunity to opt out of taking the exam, so that they do not undergo the tremendous pressure of deciding whether or not to take the exam in unsafe circumstances (circumstances that some of their peers will not be subjected to).

4) The MCAT rescheduling announcement was made exclusively on Twitter. Students without social media or sufficient internet access were left in the dark and likely ended up having to sign up for MCAT dates near the end of the cycle.

Because of all of these reasons, we ask admissions committees to seriously consider the prospect of both making the MCAT optional this year and equally considering applications with no MCAT score for admission. We also encourage medical schools to enable applicants to mask previous scores, as countless students have unfortunately been denied the opportunity to retake their MCAT due to the pandemic. These steps will be critical in assuring that we avoid further excluding URM, first generation, and low SES students during this unprecedented application cycle. Our hope is that other institutions can join Stanford in putting students and public health at the forefront of their decision making processes.
