Hello parents/guardians! Please utilize this form to let staff know that your student will absent from Lakeside Elementary School today.
Communication to excuse an absence may be turned in up to 48 hours after the missed school day.
Please follow up with the main office with any additional questions!

*After the 10th reported sick day, a doctor's note is required to excuse any absence.

*School staff may follow up by telephone if further clarification is needed to excuse the absence.
