As consideration for my child being permitted by West Valley School and the Cupertino Union School District to participate in the West Valley Valet Program, I hereby agree that I, my heirs, or legal representative, or those of my child, will not make a claim against, or bring suit against West Valley School, or the Cupertino Union School District, or any of its agents or employees, for injuries or damage resulting from the negligence or other acts, howsoever caused, by any student, parent or any person or entity as a result of my child’s participation in the West Valley Valet Program. I hereby release West Valley School and the Cupertino School District and any of its agents or employees from any actions, claims, or demands that heirs, my legal representative, or I now have, or may hereafter have for injury or damage resulting from my child’s participation in the West Valley Valet Program.

I acknowledge that my child has voluntarily applied to participate in these activities with my knowledge of the possible dangers involved, including severe personal injury, and hereby agree to accept any and all risks attendant to such participation.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between West Valley School, the Cupertino Union School District (in the city of Sunnyvale, California), and myself and sign it of my own free will.
