Liability Waiver:  I realize that participation in dance activities could involve some possible personal injury. Despite precautions, accidents and injuries can occur. By checking the box below this release waiver, I assume all risks related to the use of any and all spaces used by Saturn Jive Dance Space, Josephine Glass, Madjam), including Madison Ballroom. I agree to release and hold harmless SJDS, including its teachers and volunteers as well as facilities used by SJDS, specifically Madison Ballroom and Millkat Healing Arts, from any cause of action, claims, or demands of any nature whatsoever, now and in the future. I will not hold SJDS, its teachers, or Madison Ballroom or MillKat Healing Arts liable for any personal injury or any personal property damage, which may occur on the premises used during the classes, dances, or any related activity.                                                                                                                     Photo/Video Waiver:  Saturn Jive Dance Space (SJDS) may take photographic images and video footage during practice sessions, classes, social dances, and all related events, and reserves the right to use these photographic images and video footage as it sees fit for marketing, branding, and informational purposes without compensating the individuals in the photographs/video footage in any way. By checking the box below this release waiver, I give SJDS my permission to use any photographic images and video footage containing my person in the above mentioned manner(s), at the discretion of SJDS. I furthermore release SJDS from any injury, real or perceived, from the use of said photographic images and video footage.
