Player and Spectator Assumption of Risk and Waiver of Liability

Golf on Top is a non-profit organization to guide kids to enjoy the game of golf. In consideration for allowing the child to participate in Golf on Top, I the undersigned, hereby agree as follows:

1. Acknowledgement of Risk: I understand that participating in Golf on Top events involves risks like getting hit by a golf ball or tripping on a certain object, etc. I acknowledge that these risks may result in injury or harm to the child.

2. Responsibility: I, as the parent/guardian, assume full responsibility for any and all injuries, losses, or damages that the child may sustain while participating in any Golf on Top events

3. Release and waiver of Liability: I hereby release, discharge, and hold harmless Golf on Top, its volunteers, and their parents, from any and all liability, claims, demands, action, or cause of action whatsoever arising out of or related to any loss, damage or injury, that may be sustained by the cold while participating in Golf on Top events.

4. Insurance: I understand that Golf on Top does not provide any insurance coverage for the child's participation in any Golf on Top events. I acknowledge that it is my responsibility to provide adequate insurance coverage for the child.

5. Photographic Release: I acknowledge that my child will be in photos/video while participating in Golf on Top events. I will allow the photos/videos for promotional or informational purposes without any compensation towards the club. If you have an issue and don’t want your child to be photographed please privately contact us and it’ll be acknowledged for future purposes.

6. Participant rule: I understand that Golf on Top can remove the child from participating during the event if they show unsportsmanlike conduct activities like cussing, fighting, and other inappropriate behaviors. There will be no refunds, if they get removed from the event. It is also the parent's responsibility and will take full responsibility for any of their child's actions.

7. Refunds: I understand that Golf on Top will not refund me for any reason.

Acceptance of this Assumption of Risk and Waiver of Liability is a condition of player/parent registration. Checking the box below verifies acceptance of this condition of registration.
