By completing this application form, you consent to have your details shared with DQ Institute.

Upon the completion of this application form, you will receive the DQ Adopter Mark, which may be displayed on your website or with the program that you're applying for. There will be no evaluation on your program nor cost for the DQ Adopter mark.

If you're intending to apply for the DQ Co-Creator Seal to get your program evaluated and endorsed for its alignment with the DQ Global Standards, please complete the DQ Co-Creator Application form instead found here: https://docs.google.com/forms/d/e/1FAIpQLSfycIabN2WDSM84xre5xSeXPrR6JwYcpRNuYDDQU_x3bdOhWw/viewform, or email partnership@dqinstitute.org to find out more.
In addition to getting the DQ Adopter mark, you'll also get the DQ Co-Creator Seal and the Co-Creator Report upon the successful evaluation of your program. Please note that the cost of the DQ Co-Creator Seal is USD5000.
