Fundraiser Request Form
All fundraiser requests forms are due a minimum of two weeks prior to the proposed fundraiser start date.  PVMS administration reserves the right to deny any fundraiser requests that are in violation of board policy and/or education code.  PVMS administration strongly recommends that the purchase of any materials and supplies for each fundraiser be postponed until after the fundraiser has been officially approved.  

Any club or on-campus organization found to be operating unapproved fundraisers may lose the opportunity to fundraise in the future.
