1. I herby consent to disclosure of the above information to the VRC Executive, DOH and coach selection committee.

2. I herby acknowledge the authority of the CHA, BCAHA, VIAHA, and VRC Minor Hockey Association and agree to abide by their constitution, bylaws, rules and regulations.

3. I herby agree to familiarize myself with the National Coaching Certification Program (NCCP) requirements for coaching minor hockey and ensure that I maintain the required level of certification (see coaching requirements below).

4. I will complete the criminal record check form (code available on VRC website).

5. I agree to participate enthusiastically in VRC coach development as well as demonstrate positive and professional leadership and support for VRC teams, players, staff, parents and executive at all times.

6. Failure to comply with items 1-5 may result in dismissal from any coaching role that may be granted.
