STEP #1 - Complete the following form to be registered for Camp House 2019! (One registration form per person attending camp.)
STEP #2 - Pay $50 non-refundable deposit. Please drop off your deposit at either Paynesville Lutheran Church office or with Dave Long at United Methodist Church of Anoka. The remaining balance is due on or before Friday, May 31st, 2019.
CAMP OPTIONS CREW/CONFIRMATION (Completed 6th-8th grade) - $300 Monday July 22nd - Saturday July 27th, 2019 PEER Ministry (Completed 9th-12th grade) - $400 Friday July 19th - Saturday July 27th, 2019 PMT PEER Ministry Team (Completed PEER Ministry) - $250 Friday July 19th - Saturday July 27th, 2019 Young Adult Volunteers (Completed 1 year post-High School) - $30 a night + $10 for T-shirt Flexible dates between Friday July 19th - Saturday July 27th, 2019 Volunteers/Families - $30 a night + $10 for T-shirt Flexible dates between Friday July 19th - Saturday July 27th, 2019 *NOTE: We want everyone who would like to attend camp to be able to go. If you are in need of financial support let us know - this request will be confidential. Camp is such a wonderful experience and we want you there!
STEP #3 - Complete a health form. Health forms can be found on the www.paynesvillelutheran.org website or can be e-mailed to you by request from Dave Long (email@example.com).
