This membership application and renewal form is intended for KRI certified Kundalini Yoga teachers or support persons who are interested in furthering the mission and goals of KY-WHA by contributing 1-5 hours a month of service developing, promoting and/or teaching KY-WHA programs, and for those who strongly support any KY-WHA program, such as Beyond Addiction. The core purpose of Kundalini Yoga Wellness and Health Association (KY-WHA) is to deliver customized health-oriented Kundalini Yoga programs to the health care, educational, corporate and arts communities and general public. KY-WHA was established as a non-profit association in 2008. Membership responsibilities include "live" or virtual attendance at 8-12 KY-WHA meetings annually (for General Members) or attendance at 4 meetings annually that relate to the KY-WHA specialty program you teach (currently Beyond Addiction is the only approved KY-WHA program). These meetings are essential to keep us working harmoniously and effectively together to spread the teachings of Kundalini Yoga in a professional manner.

KUNDALINI WELLNESS AND HEALTH ASSOCIATION (KY-WHA) MEMBERSHIP POLICY
Membership
1. Membership in the Association shall be open to those persons interested in UPHOLDING and FURTHERING the objects of the Association whose application for admission as a member has received the approval of the Board of Directors of the Association.
2. Membership shall consist of FOUR (4) classes being Regular, Lifetime, Associate and Honorary Membership as follows:

1. Regular Membership shall be available to those that meet the following criteria:
a) EIGHTEEN (18) years of age or older
b) certified Kundalini Yoga instructors trained in the teachings of Yogi Bhajan.
c) submission and approval of application by the Board of Directors
d) complete payment of the annual Regular Membership fee.
e) the Board may request a police clearance letter or reference check* at the member’s own cost
f) ability to attend monthly meetings in person or virtually (at least 8/year)
g) contribute at least 1-5 hours a month of service to fulfill the goals, tasks and projects of KY-WHA
h) commitment to one or more of the Service Teams listed on the registration form

2. Associate Membership shall be available to those that meet the following criteria:
a) all persons under the age of EIGHTEEN (18) years of age (or over 18)
b) certified Kundalini Yoga instructors trained in the philosophies of Yogi Bhajan.
c) submission and approval of application by the Board of Directors
d) complete payment of the prescribed annual Associate Membership fee.
e) ability to attend at least 4 meetings per year in person or virtually
f) contribute at least 1-5 hours of service every three months to fulfill the goals, tasks and projects of KY-WHA
g) commitment to serve one or more committees listed on the registration form

3. Lifetime Membership shall be available to any Member by:
a) application for approval submitted to the Board of Directors and, upon approval, payment of the prescribed Lifetime Membership fee.

4. Honorary Membership may be granted to any person by the Board of Directors at the invitation of the Board. Honorary Members shall not be required to pay membership fees. Honorary Members shall be entitled to attend all annual or other general meetings of the Association but shall not be entitled to hold office or vote.
a) An honorary member may include, but not limited to: health professionals who are passionate about the benefits of KY and its effects on individuals’ mental, physical and spiritual well-being
b) members of the KY community we wish to invite to be members of this association.

Membership Termination
Members of all classes of membership may withdraw or terminate membership in the Association upon written notice to the Association delivered in person, by email or by ordinary mail addressed to the head office of the Association. Such notice shall take effect immediately upon personal delivery of such notice and in the event of delivery by ordinary mail, two (2) business days from the date of posting.

a) Any member of any class may be required to resign from the Association by a vote of three-quarters (3/4) of the members.

j) Opportunity to participate in and benefit from fundraising events and seek financial opportunities as an association i.e. grants, yoga demonstrations etc
k) Opportunity to list KY-WHA, Beyond Addiction websites on your email outgoing messages along with advertising any upcoming workshops
l) Opportunity for you to be listed on the above websites if you are an approved teacher of these programs
m) All members (regular/associate/lifetime) are entitled to access to the google sites
n) Entitled to vote at any annual or general meeting of the Association.

Associate Member Benefits
a) Opportunity to participate in any KY-WHA Curriculum, and be considered for certification as an instructor or trainer of specialty programs after meeting the prescribed criteria
b) Use of promotional brochures for a fee
c) Welcome to attend any annual or other general meeting of the Association, but must attend 4 pre-set meetings per year in person or virtually to stay abreast of changes and policies.
d) Access to meeting minutes through the Google site, and the expectation is that you will read them monthly
e) Associate members do not participate in program development.
f) For an associate member to train teachers of any KY-WHA program, the member must take the course and teach a portion of the teacher training three times under the supervision of a “regular” member in order to be considered qualified and certified to train other teachers.
g) To teach a program, the associate member must fulfill the competencies specified for that program, assessed by the program originator and the KY-WHA “competency committee”
h) Have your name, contact details and photos on promotional material including, but not limited to website, brochures, flyers etc

Membership Responsibilities

Regular Member Responsibilities
a) A commitment of at least 1-5 hours a month by the member to participate in a “To do” list posted on the Google site. Members can have a choice in jobs listed on the registration form.
b) Be available to develop and/or train and deliver a variety of KY-WHA curriculum and meet with potential clients
c) Notify membership coordinators (secretary and treasurer) of any change in information, i.e. contact details or new curriculums you can teach
d) Payment of yearly membership fee promptly
e) Attend approximately one KY-WHA meeting per month or eight per year
f) Fulfill the commitments and competencies as needed to teach programs

Associate and Lifetime Member Responsibilities
a) A commitment of at least 1-3 hours every months by the member to participate in a “To Do’ list or committee work generated from the Association meetings.
b) Attendance for at least 4 predetermined meetings annually
c) Reading of the minutes from all monthly meetings
d) Contributing ideas and assistance wherever possible
e) Payment of Annual membership fee
f) Fulfill the commitments and competencies as needed to teach programs

Meeting Attendance/Responsibilities
a) Meeting dates are given in writing for the year beginning in January. The 2017 meeting dates are

Dec 3, all at 7:00 a.m., Toronto time. Please mark these in your calendar now. The 4 mandatory meetings for associate members are Feb 12, Jun 11, Oct 15 and Dec 3.
b) Members are expected to either attend the meeting in person, or virtually (i.e. on Zoom) or read the minutes before the next meeting
c) Regular members cannot miss two meetings in a row or more than three meetings in a year unless there are extenuating circumstances. Should this be the case, membership is reviewed by the board (Board will take into consideration the amount of work and input the member has over and above attending the meetings)
d) Associate members are required to attend 4 meetings per year
e) Notify members if transportation is an issue in the ability to attend meetings; carpooling may be an option, or attending virtually
f) If a member is unable to attend a meeting, they are expected to send regrets to the secretary
g) Members are expected to read policies and minutes to keep informed of the progress and happenings of the business.
h) Check email regularly, especially the week before a meeting for updates, minutes and changes to the agenda, date/time or location of meeting
i) Members are invited to submit items to the secretary to add to the agenda 1 week prior to each meeting

Membership Fees
a) Membership fees are renewed by Jan 31 each year
b) Dues are not prorated if joining during the year
c) Dues are non-refundable.
d) Reminders for payment are sent out in Dec. There is a grace period of one month,
e) If payment is not received by Jan 31, then the membership is terminated
f) Membership fees are allotted to maintenance costs, publicity, promotional materials, health shows etc, and ensures commitment of members
g) The secretary and treasurer are together responsible for administering membership invitations, forms and payments, and may share these tasks with the Assistant Secretary and/or Assistant Treasurer, when such members are in place on the Board.

Regular Membership Fee
a) Membership fees are the same for both Regular and Associate members - $60.00 + TAX in Canada, and an adjusted amount in other countries, based on the currency conversion rate. The Board maintains a fee schedule for all participating countries, based on the impartial international minimum wage Purchasing Power Parity (PPP) figure, in ratio to the Canadian membership fee, plus current banking fee, payable in Canadian dollars. HST must also be collected from members living in Ontario, Canada, by provincial law, being the province of the address of the Corporation.
b) If a member does not attend meetings and there is no payment or notification intent to renew by Jan 31, then member is contacted twice and removed from the list if there is no response, and their listings on any websites associated with KY-WHA are removed.

Associate Member Fee
a) Membership fee is $60 + TAX or Purchasing Power Parity amount
b) No follow-up is made if the membership lapses

Lifetime Membership Fee
a) Can be Regular or Associate
b) Membership Fee is $1000.00 + TAX
c) Non refundable

MEMBERS’ MEETINGS
a) The Annual or any other general meeting of the members shall be held at the head office of the Association or at any place in Canada as the Board of Directors may determine and on such a day as the directors shall appoint. The members may resolve that any particular meeting of members be held outside Canada (ie at Summer Solstice in New Mexico) or online via Zoom or Skype or Google Hangout.
b) At every annual meeting, in addition to any other business that may be transacted, the report of the directors, the financial statement shall be presented. The members may consider and transact any business either special or general at any meeting of the members. The Board of Directors or the president or vice-president shall have power to call, at any time, a general meeting of the members of the Association. The board of directors or President shall call a special general meeting of members on written requisition of members carrying not less than FIVE (5%) percent of the voting rights. ONE THIRD (l/3) of the members present in person at a meeting will constitute a quorum.
c) FOURTEEN (14) days written notice shall be given to each voting member of any annual or special general meeting of members. Notice of any meeting where special business will be transacted shall contain sufficient information to permit the member to form a reasoned judgment on the decision to be taken. Notice of each meeting of members must remind the member that he or she has the right to vote by proxy.
d) Each voting member present at a meeting shall have the right to exercise one vote. A member may, by means of a written proxy, appoint a proxy holder to attend and act at a specific meeting of members, in the manner and to the extent authorized by the proxy. A proxy holder must be a member of the Association.
e) A majority of the votes cast by the members present and carrying voting rights shall determine the questions in meetings except where the vote or consent of a greater number of members is required by the Act or these by-laws.
f) A resolution in writing, signed by all the members entitled to vote on that resolution at a meeting of members, is as valid as if it had been passed at a meeting of members.
g) No error or omission in giving notice of any annual or general meeting or any adjourned meeting, whether annual or general, of the members of the Association shall invalidate such meeting or make void any proceedings taken thereat and any member may at any time waive notice of any such meeting and may ratify, approve and confirm any or all proceedings taken or had thereat. For the purpose of sending notice to any member, director or officer for any meeting or otherwise, the address of the member, director or officer shall be his or her last address recorded on the books of the Association.
h) A majority of votes cast on any resolution shall determine its resolve unless otherwise provided in the Act or these By-Laws.

Conflict Resolution:
a) If any member has a conflict of interest with respect to a matter to which they are assigned, that member will immediately advise the Chairperson of the Board of such conflict.
b) The chairperson will advise the member to disclose the conflict to the rest of the board, to allow the full board to decide whether they wish to waive any concerns with respect to the conflict.
c) It is the responsibility of the member to disclose a conflict at the earliest possible opportunity.
d) If the member has questions as to whether a conflict exists, it is the responsibility of the member to raise the matter with the chairperson of the Board at the earliest opportunity in order to determine the course of action.

Resignation:
a) Once a member resigns(s) he is no longer involved in curriculum development. The member’s name will remain as an author on any curriculum developed, or under development, to which the member has contributed.

If the conflict arises in/outside of the meeting between a few members then a mediator is agreed upon by all parties and a separate meeting is set to resolve the conflict.
