EVERY PERSON WHO PARTICIPATES IN CURLING AT THE MINOT CURLING CLUB NEEDS TO COMPLETE THIS FORM AND AGREE TO THE LIABILITY WAIVER. NO EXCEPTIONS. 

We do not share the personal information provided, and retain it solely for records necessary for our insurance. 

This form can also be used to elect and pay your annual membership if you choose that option. For those paying weekly you will complete payment in person at the club, we accept cash, check, card, and other electronic payment options (online via square). You are not required to pay when completing the form. 

At the end of the form, if you elected to pay an individual annual rate you will be linked to a payment portal to complete payment. Team annual rates will be accepted at the club so we can verify team members and for mixed teams you will pay a prorated amount based on the mix of the team, see Wes in person or reach out by phone 218-766-1988.

For anyone participating in a learn to curl or other event where payment is not required, please select that option in the dues questionnaire.

Reminder that recorded matches with participants who are not in good standing monetarily with the club maybe subject to forfeit.
