Product Recall Form for Power Bank

URGENT – IMPORTANT PRODUCT SAFETY RECALL

Spector & Co. is voluntarily recalling the T119 Elf 2,200mAh Power Bank (the “Power Bank”) in cooperation with Health Canada and the United States Consumer Product Safety Commission. During charging or discharging, the unit can over heat posing a fire hazard. If you are in possession of such a Power Bank, stop using it immediately. As set forth in detail below you should not send the device through the mail or throw away the device.

The Power Bank is rectangular portable battery-powered device for charging the battery of a mobile phone or other electronic device. The unit measures approximately 4” (11 cm) long by ¾” (2cm) high by ¾” (2cm) wide. It has a color match end with a wire key loop and the sides are either Gun Metal (GMT) Blue (BLU) Black (BLK) & Red (RED). On the black top of the charger in white letters are "5V OUT" and "5V IN". The name and/or logo of the organization that gave away the unit as a promotional item appears on the side of the power bank. The unit was packaged in a black carton and given away as promotional items at various meetings, trade shows and industry conventions from May 2016 to December 2018.

The Power Bank contains a lithium-ion battery and must be disposed of in accordance with your local, provincial, state or federal regulations for household hazardous waste. You can learn more about the appropriate drop off sites in your area by contacting your local environmental, health or solid waste agency. Prior to dropping off your Power Bank at such site you should confirm that the site will accept recalled lithium ion batteries. Do not send the device through the mail and do not throw away the device.

Disposal of the Power Bank

To be eligible for a replacement Power Bank, you are required to first dispose the Power Bank as described above in accordance with your local, provincial, state or federal regulations for household hazardous waste. When applicable, Spector & Co. will cover the direct costs of disposal of any units that may be charged by the facility handling the disposal of the Power Bank. To receive any such reimbursement along with a replacement Power Pack, you will need to provide a receipt from the disposal/recycling facility.

General Conditions

To be eligible for a replacement Power Pack, you must complete and sign the present Product Recall Form provided by Spector & Co. The Product Recall form must also be sent to Spector & Co following the guidelines described herein.

To be eligible for a refund of the direct costs associated with disposing of the Power Bank, you must include with the completed Product Recall Form the receipt obtained from the disposal/recycling facility showing disposal of the Power Bank and payment.

Any omission, misrepresentation or fraud will result in the refusal to process your request for a replacement Power Bank.

That you release and hold harmless Spector & Co from any and all liability for any damages, claims or prejudice of any nature whatsoever that may result from your continued use and/or non-compliant disposal of the Power Bank.
