I hereby authorize the Symphony Orchestra of North Georgia (SONG) to release the member’s photographs and videos to the news media, on www.symphonynorthgeorgia.org and all social media. Please note: For the Youth Orchestra Exhibition, photographs and videos are taken in group settings. If you would like us to exclude your child from individual photos, please indicate that in the comments section.
 
Additionally, I give permission to the SONG staff to authorize medical treatment for the student member in case of an emergency during involvement in any SONG event or activity. I agree that neither I/the student member, nor anyone acting on the student member's behalf, will sue the Symphony Orchestra of North Georgia, SONG directors, officers, employees or agents for or on account of personal injury, loss of health, loss of property, inconvenience, delay, or other damage sustained directly or indirectly by the student member resulting from the student member’s involvement in the Symphony Orchestra of North Georgia, and I agree to hold harmless and indemnify the SONG and said other persons from and against any and all liabilities and expenses which the SONG may incur in respect to any claim, suit, or cause of action on account of any such injury, loss, or damage. In case any provision of this release shall be invalid, the validity and enforceability of the remaining provisions shall not be affected or impaired.
