MIDDLE SCHOOL, HIGH SCHOOL and CALA REGISTRATIONS HAVE CLOSED
If you wish to remain with your current in-person or distance learning selection, please do not complete this form.

Please complete this form by October 23rd for grades 5-12 and by October 30th for PK-4 only if you wish to change your student's current registration from in-person learning to distance learning or distance learning to in-person learning. This choice will be for Quarter 2 and Quarter 3 for grades 5-12 students and for Trimester 2 for grades PK-4.

Please note, we ask that you include the student number and school of each child in this request. You can find the student number by logging in to the PowerSchool parent portal - https://ps.chariho.k12.ri.us/public

The student number is next to the name of your student at the top of the page.

Please note once we receive the registrations we will be re-evaluating our plan to determine any revisions to our plan.
For your reference Quarter 2 begins November 23rd and Trimester 2 begins December 14th.
