MLQ Player Disciplinary Review Form
The MLQ Player Disciplinary Procedures are meant to discourage poor behavior among members of our league. Any incidents that take place during MLQ events must be reported via this MLQ Player Disciplinary Review Form within two (2) days following the occurrence. Submission should include pertinent details of the incident. This includes which game or practice the incident occurred in as well as the names of any involved parties. We discourage members from submitting disciplinary reports without due cause. Submissions that do not supply due cause will not be reviewed.

If an incident does not occur during an MLQ event or falls under the purview of the MLQ Harassment Policy, all matters will be reviewed by the MLQ People Operations Director and are subject to the MLQ Harassment Policy.
