Note that:
1. No refund will be issued for a monthly plan cancelled 'immediately' (Option 1) during the very first month of the plan. Such requests will instead be treated as requests to cancel at the end of your current billing period (Option 2).
2. You cannot use this form to cancel or refund a purchase of an EventForm plan for a particular event.
