MODOC COUNTY FAIR QUEEN AND COURT APPLICATION
This application must be completed in ink or with a computer and must be received at the Modoc District Fairgrounds Office no later than 4:00 p.m. on Friday, May 31, 2024.  Applications that are received later will be considered ineligible.  Please use additional pages if necessary.  If you are currently still in high school, please attach a copy of your most recent report card.

If the application is completed in Google Forms, please be sure to still print and sign the APPLICATION AGREEMENT. The APPLICATION AGREEMENT can be found on the last page of the MODOC COUNTY FAIR QUEEN AND COURT APPLICATION. Complete applications include the following documents:
Application, Personal Emergency Information, Waiver of Liability, Newspaper Biography and Modeling Information.
