Center for Creative Arts 2024 Summer Camp Enrollment Form 

Please use this form to Register for Summer Camps. Please submit a separate form for each child, and indicate which camps you would like to enroll in. 

Our Financial Administrator will email you an invoice that must be paid online within 2 weeks of invoice receipt to secure your registration. Art camp class size is limited to 14; Robotics limited to 12; Musical enrollment is limited to 35 students in each week.  Camps will fill up quickly! Spots are reserved through time-stamped receipt of this form. If you have any questions, please email us at center4creativearts@comcast.net

Cancellations: Prior to May 15th - fully refundable. After May 15 and before June 1st - refundable minus $50. No refunds after June 1st.

Please note The Center does not administer medication of any kind, except for the use of an EpiPen and / or Benadryl that is clearly labeled and brought to the Center by the parent of the child with a life-threatening allergy, with written instructions. Please contact The Center to request our Life-Threatening Allergy Action Plan form.

Policies and Procedures Agreement

Payment Policy
  • Please finalize payment within ONE WEEK of receiving emailed invoice . We also accept cash or check at The Center office during our open hours: Monday - Thursday 3:30 - 7:30 pm, at 4825 Pouncey Tract Road, Glen Allen, 23059.
  • Due to limited class size, non-received payments (within one week) will result in your child being moved to the waiting list. 
 Cancellation Policy

  • For closings due to inclement weather, please call the Center at (804) 360-2687 and check for emails.
  • The Center for Creative Arts reserves the right to cancel any programs for which there is insufficient registration with a full refund.   
Publication Release 
  • In an effort to share the story and ministry of The Center for Creative Arts at Shady Grove United Methodist Church, photographs, video, and audio recordings of students may be used in promotional materials which include, but are not limited to, brochures, newspaper, or magazine articles, DVDs, the Center’s and/or the church’s website. Please be assured that minors will not be identified by name in any publication. 
  • By agreeing to this release, I authorize the Center for Creative Arts at Shady Grove to publish and release photographs or recordings of my student and state that I understand that neither I nor my family will receive any form of compensation from said publication. I also hereby release the Center for Creative Arts at Shady Grove from all claims of every kind on account of such use. 
General 
  • If a student becomes a threat to themselves or others, the Director has authority to send the student home. Continuation in the program will be reviewed on an individual basis. 
  • The Center for Creative Arts has an open door policy - parents are welcome to visit classes at any time, and are encouraged to discuss goals and concerns with the instructors and the Director at any time.
