Change of Program Submission Form
Purpose: Use this form to change or add an academic major, minor, and/or concentration. You must be an active student in a current degree program to use this form. It is strongly recommended that students consult with the advising center or a faculty advisor prior to completing this form.  

Changing from a bachelors degree to an associates degree or certificate could impact your financial aid eligibility. Students are strongly encouraged to contact UMAFA@maine.edu if they plan to do so.

Students changing their major to one that leads to licensure should visit the UMS State Authorization & Licensure page to learn more about their specific program and the licensure requirements in other states and territories, in addition to the contact information needed to inquire further into the licensure requirements associated with this program.

Effective Semester and Academic year of Requested Change:
Timeliness of Form: In order for the change to be applicable for the selected term, forms must be received AND processed by the Advising Center no later than February 14th for the spring term and October 14th for the fall term. Changes requested after these dates will become effective for the following term.  For example: Fall 2023 requests after Oct. 14 will go into effect for Spring 2024. It can take up to two weeks for the Advising Office to process Change of Programs. The effective date is based on when they are processed, not when they are submitted, so plan early enough in advance to get the effective semester you want.
Is this change required for Financial Aid?
Financial Aid: Changing from a baccalaureate degree to an associate degree or from an associate degree to a certificate program may affect your eligibility to receive financial aid. Students receiving – or planning on receiving – Federal Financial Aid (Pell Grants) MUST speak with the Financial Aid Office for details prior to submitting this form.
