COPPA Parental Consent Authorization Form for Grades4life
To provide a secure and educationally beneficial experience for your child, our school/organization intends to use Grades4life (the “Platform”), a digital reward system and virtual school store, to support student achievements and motivate learning. The Platform complies with the Children’s Online Privacy Protection Act (COPPA), which requires parental consent for the online collection of personal information from children under 13. As a school, we are permitted by COPPA to act as an intermediary to provide consent on behalf of parents for educational services.
What Information Will Be Collected and How It Will Be Used
Grades4life will collect limited information from students, such as their name, student ID, and achievement data, to:
- Track student progress and award points based on academic achievements.
- Allow students to redeem earned points within the virtual school store.
