Essential International Travel - Approval Request Form
This document is intended to be used by CCAS faculty, staff, and graduate students who are required to obtain approval from the Dean's Office before any international travel can occur.

Pre-step: In accordance with GW’s Interim Essential International Travel Policy, (https://global.gwu.edu/interim-policy-international-travel) the CCAS Dean's Office must approve any proposed GW-affiliated travel. Before completing the Essential International Travel Approval Request Form, please review the Essential Travel Acknowledgement Document (https://global.gwu.edu/acknowledgement-travel-responsibilities-and-limitations-support), which all travelers will be expected to sign and abide by for any GW-affiliated travel. If you are unable or unwilling to comply with the information included in the Essential Travel Acknowledgement Document, your travel plan will not be approved.

Step 1: Please complete this form to provide some basic and critical information regarding your future international travel. If your travel is approved as essential, you will be asked to provide a detailed risk mitigation plan (Step 2). In this section, please only include a brief overview of how the country is addressing and/or you are preparing for existing health and safety concerns.

Step 2: If your request is approved for consideration through Step 1, will you be provided with another form, Essential Travel Risk Awareness and Mitigation Questionnaire, to complete Step 2. At this stage, we need to review a proposal with detailed information on risk awareness and contingency planning to determine the viability of the travel plan.

CCAS Dean's Office will communicate the decision after reviewing your detailed proposal.
