By completing this form, I, as the student's parent or guardian, am certifying that I am personally transporting the student named below, or have arranged transportation with an adult (non-student) of my choosing for this student. I understand that the Clinton Public School Athletic Rules require that students ride the buses to and from all athletic events and a departure from this requirement will release Clinton Public School from all liability for any adverse results that may occur. I agree to release the Clinton Public School and its employees and officers from all liability with reference to the below stated transportation.

Any private vehicle used for the transportation of pupils must be owned by the approved driver or the spouse of the approved driver; have the capacity to hold not more than eight persons; and must conform to registration, inspection, and insurance requirements of the State of New Jersey for privately owned vehicles.  Seat belts shall be worn by the driver and the passengers while the vehicle is in motion.  No vehicle may be used to transport more persons than its normal load capacity.
