Record of Fire drill practice/Evaluation

Fire drill evaluation must be completed every 3 months in a registered child care service. The below evaluation form has been designed for Ignite Minds to assist you practice and improve your evacuation.

It is important you answer the questions accurately. It is ok if you do not meet all the requirements in a practice run as long as you learn and take steps to improve. After all, the whole point of a practice run is to prepare you for any real emergency.

You do not need to answer the questions relating to 'bush fire at risk' zones unless relevant to your service - if unsure you should ask. All other questions marked * are required. You will be emailed a copy of your completed form when you submit at the end.
