Record of Emergency drill practice/Evaluation

Emergency drill evaluation must be completed every 3 months in a registered child care service. The below evaluation form has been designed for Ignite Minds to assist you practice and improve your evacuation.

It is important you answer the questions accurately. It is ok if you do not meet all the requirements in a practice run as long as you learn and take steps to improve. After all, the whole point of a practice run is to prepare you for any real emergency.

You do not need to answer the questions relating to 'bush fire at risk' zones unless relevant to your service - if unsure you should ask. All other questions marked * are required. You will be emailed a copy of your completed form when you submit at the end.

The following questions relate to your practice drill *
Is your evacuation plan displayed clearly and prominently at all exits?
Did you follow your evacuation plan?
Was your drill easy to follow and implement?
Was your Evacuation bag ready and stocked appropriately?
Children were calm, orderly and cooperative?
Is the evacuation point still a safe and appropriate place to go to?
Are you clear where you would go if you immediate danger is identified and it is determined that the children should be secured inside the building for their own safety (lock down room)
Are you clear on what circumstances would trigger a 'lock down'
Could your evacuation drill be improved?
If you answered 'no' to any of the above, detail anything you observed that you will do differently next time to ensure that an emergency is handled better?
The following questions relate to your evacuation bag *
Did you have access to snacks and water
Is your First aid kit stocked and up to date?
Did you have access to medication if required?
Did you have access to all Medical contacts?
Did you have access to all parent/guardian numbers?
Did you have access to all emergency numbers?
Did you have access to spare nappies, wipes and gloves.
Does your Evacuation Bag need updating?
If you answered 'no' to any of the above, detail anything you need to do to ensure your evacuation bag is up to date.
The following questions apply if you are in a designated bush fire at risk zone

Have you completed the CFA fire Ready Kit?
Have all your parents signed the Ignite Minds emergency management plan?
Does your evacuation bag include a fire blanket to shelter from embers?
Do you have a battery operated radio to access ABC emergency updates?
Is your VIC Emergency app set with a 20km incident watch zone?
Do you have disposable respirator masks for children?
Was your vehicle ready with child restraints fitted correctly?
Have you communicated your leave early plan
Do you have a plan if it is too late to leave.
Clear selection
Have you met your statutory obligations for bush fire at risk zones?
If you answered 'no' to any of the above, detail anything you need to do to meet your requirements?
A copy of your responses will be emailed to the address you provided.
