Thank you for referring an older adult to Little Brothers Friends of the Elderly - San Francisco Chapter.
Please read the following before submitting your referral form. We also ask that you mention Little Brothers Friends of the Elderly to the older adult you are referring so that they can be on the look out for more information and phone calls from us. All of our programs are free of charge.
To participate in our programs, an older adult must be: 65+ years old OR 60+ years old and living with a mobility disability that leaves them bedbound,
live alone in San Francisco,
and receive less than two social visits per month. (Healthcare providers and social workers and/or case managers are NOT considered social visits.)
