Please complete this form about your playing history to accept your Women's Division Hall of Fame nomination. This information will be used by the Peer Pool to evaluate candidates for placement on the Hall of Fame slate. 

Eligibility requirements: nominees are age 38 or older/1987 or earlier birth year 

Once you complete the Nominee Information form, please email a headshot photo and an action shot (in JPEG or PNG format, including the photographer's name, if available).

At the end of Peer Pool voting, you will be notified of your standing and informed if any additional information is needed.
