    Field Trip Permission

    There will be instances throughout the year where students will travel off campus, during regularly scheduled program time, with or without school staff, for learning experiences related to their coursework. If the student is not accompanied by school staff, they will be under the supervision of an adult during work based or a job shadow experience that has been previously scheduled. The nature of the programming available here at NWTC lends itself to hands on learning and lifelong learning. One of the benefits of Career and Technical Education (CTE) is that students see the relevance and connections between school and work. An example of these area excursions include but are not limited to: job shadows, work based learning, industry site visits, career and job fair expositions, and legislature round table discussions. If a field trip should arise outside of regularly scheduled program time, you will be notified in advance and asked special permission. These outings may be on foot, by bus, or car. If by car, no students shall drive other students, and adult drivers have met district policy standards. The school nurse will not be available on any of the above described trips. ALL SCHOOL AND DISTRICT RULES, REGULATIONS, AND POLICIES ARE IN EFFECT DURING FIELD TRIPS.

    Medical/Emergency Information/Informed Consent Form

    Please be sure the school nurse has any medication(s) your child may require. If necessary, the school nurse will designate an adult on the trip to carry and dispense any and all medications needed during the field trip. CONTACT THE SCHOOL IMMEDIATELY WITH ANY CHANGES IN INFORMATION. As the parent/guardian of the above named student, I authorize qualified emergency medical professionals to examine, and in the event of injury or serious illness, administer emergency care, including transport, to the above named student. I understand every effort will be made to contact me to explain the nature of the problem prior to any involved treatment. In the event it becomes necessary for the school district staff-in-charge to obtain emergency care for my student, neither he/she nor the school district assumes financial liability for expenses incurred because of the accident, injury, illness and/or unforeseen circumstances. These activities are an extension of the school education program and student conduct is to be in accordance with the school's published rules and regulations.
