- Designated Block Captain to complete the form (must be a resident of the street)
- Verify that you have emailed your completed letter of support to
phoenixday@livablecity.org.
- Confirm the address where you will receive barricades on Wednesday, October 12th
- Certify that you have read the program guidelines and agree to adhere to all of them.
