First Grade Yearly Permission Slip
Please review and sign the permission slip below in order for your child to participate in the fun educational activities we have planned throughout this school year.  Scheduled events and dates listed below may be subject to change.  They are NOT limited to, but will include the following:

Field Trips:  Hogle Zoo tentatively scheduled for 9/12/2017, Ogden Nature Center on-site/American Symbols 10/18/2017, Hill Aerospace Museum 5/15/2018, Ogden Nature Center/Raptors and Reptiles 5/21/2018.  
**We ask for a $15 donation to cover the cost of all the field trips payable to the office.
**All students need to bring a disposable sack lunch with them at the field trip.

1st Grade Parties/Activities:  10/31/2017 Halloween Party, 12/15/2017 Winter Party, 2/1/2018 100th Day Celebration, 2/14/2018 Valentine's Pary, 3/16/2018 St. Patrick's Day Celebration, First Day Jitter Activities including Jitter Juice (Hawaiian Punch), Marshmallow STEM Project, Graham Cracker House Decorating, Create your own Wonka Candy, Sink or Swim Cookie Experiment, End of Year Activities.
**We ask a $12 donation to cover supplies, treats, drinks, & activity supplies for the parties payable to the office.

We also will have a parent expert demonstrate the process of cheese making from farm to table, date TBD.

Movies rated G with Educational Activities:  Ramona and Beezus, Charlie and the Chocolate Factory, Polar Express, The Tale of Despereaux, and Charlotte's Web.

Patriotic Program:  5/18/2018 Afternoon School Assembly and Parent Performance the same night at 6pm.

**By e-signing this form, I agree to allow my student to participate in the activities listed above.  I agree to allow my child to partake in sugary treats and drinks for the above mentioned activities.  I also agree to pay the donation to cover these activities to the office.  Also, by e-signing this permission slip, you are agreeing to the same electronic usage form signed for the office that allows your child to use our electronic technology tools appropriately, such as lab computers, laptop classroom computers, desktop computers, iPads, iPods, etc.  If any damage occurs to these tools while in your students use you will be responsible for the repair or replacement costs.
