Fill in this form after you place an order on the Shady Ideas website. Please note that there are 3 different elements that you have to choose in terms of color, for your nameplate. The first is the background color of your nameplate. The second is the color of the motif. The third is the color of the text that you want mentioned. Please make sure that you choose all 3 of these colors.
