(it will also help if you can qualify them with respect to your feet if they aren't normal feet. Ex., Motion Control Shoes, orthotics  for flat feet, etc)

What are your best times so far for a 5k, 10K, 21K? It does not matter if you have walked thru these races. I need some baseline to start. It will also help if you put your answers to Q1, 4 and 6 above in perspective as in -> I was 25 yrs old and 60 kg when I ran 10km in 70 mins. Also where did you run your 5K, 10K, 21K? (treadmill, road race, trail race?) *

How much time can you realistically spare towards training every week? Answer day-wise if you can *

say 2 hours, 30 minutes for a half marathon
Do you have any health conditions which require you to take regular medication? *
(blood pressure, thyroid, depression, insomnia, etc). You will have to sign an indemnity before you start training regardless of how healthy you are. But this is just to ensure you have checked with your doctor and know your body better before you start
Have you had any injuries in the past (running-related or otherwise)? *
(shin splints, plantar fasciitis, runner's knee, achilles tendinopathy, calf tears, muscle strains, ITB pain, fractures, etc)
Do you participate in any other sport or physical activity currently (other than running)? If so, please give us some idea of your involvement *
(Ex: cycling, rowing, swimming, badminton, squash, trekking/hiking etc. I cycle about 3 days a week to work, 10 km each way, etc)

(You may call me "Pinky" too ;) But seriously, it will help me understand why you want to do this. A brief introduction to yourself is also useful. Do you smoke, etc? Some people come to me, saying "I don't care what happens in the race. But I want to be fitter 12 weeks from the day I start training. "I am running this race for my dad, etc". As  you will appreciate, motivation is incredibly important to your training. If you share your goals with me, I will try my very best to help you get there. It is not much fun to discover that you start training for 4 weeks and then find out you were waiting to hear from a recruiter who you met after you started training. If I know that you have some prior commitments and some constraints, it helps plan training better
