Portland Public Schools is committed to providing a safe learning environment for all students and staff. Any student, parent or staff member can submit an informal report here and we will share your report with the proper school or district contact, completely ANONYMOUSLY—none of the information you provide on this form will be traced back to you if you do not want to give your name. Your safety is the most important thing to us!

This form may be used to report ALL forms of discrimination and harassment, including sexual harassment, assault or misconduct, that occurs in the Portland Public Schools.  The form applies for incidents involving students, staff, or both, and can be utilized by any member of the PPS community.

IMPORTANT NOTICE, READ ME
Portland Public Schools may not be able to investigate or respond to anonymous reports made through this form depending on the specific allegations made; however, in cases where the identity is known, the victim/survivor may be contacted for further information and to determine whater a formal complaint will be filed.  If you choose to share your name, this may begin an investigation into your case, and may involve sharing this information with local police and notifying the parents of the individuals involved.

IF YOU CHOOSE TO SHARE YOUR NAME, THE SCHOOL ADMINISTRATION AND STAFF ARE LEGALLY REQUIRED TO REPORT ANY CASES OF ABUSE, SEXUAL ASSAULT AND VIOLENCE TO THE DEPARTMENT OF HEALTH AND HUMAN SERVICES (IF UNDER 18).

The information from these reports will also be collected and used to address overall school climate issues within the district.

If you would like to make a formal report to begin an investigation and receive accommodations in the classroom, please speak to the district or school level Title IX Liaisons, a school administrator or other support staff at your school.  The Liaisons (listed below) are there to support you and help you move forward in the safest and most effective way possible.
