Volunteers under 18 must be accompanied by a parent or legal guardian who agrees to be within eyesight and earshot of the minor they are overseeing for the event on January 11th, 2025. Without the parent or guardian confirming responsibility on this , the minor will not be able to volunteer. All adults and parents or legal guardians of each minor must read and agree to the following:

By signing this form, I, the parent or legal guardian of the minor named below, consent to the child’s participation in the volunteer activities organized by the Celebration of Lights Inc. and commit to being within eyesight and earshot of all the minors named below, who will be under my care during this event.  *
I understand that myself and the child(ren) will be provided with orientation and training necessary for the safe and responsible performance of the volunteer duties. I understand that myself or my child(ren) will receive no monetary compensation for this work.

I also understand that inherent risks may be associated with volunteer activities, including but not limited to broken bones, concussions, sprains, burns, electrocution, paralysis and death, and will not hold the Celebration of Lights Inc. accountable or liable for any injuries that unintentionally result from my or my child(ren)'s participation, or that arise during the time spent volunteering due to any underlying physical condition
