Montrose Community Schools Volunteer Application and Background Check
For the protection of the children in the school, the District is required by law to inquire of its staff members whether or not they have ever been convicted of a crime related to children. Accordingly, all applicants will be screened through the Sex Offenders Registry (SOR), the Internet Criminal History Access Tool (ICHAT), and the offender Tracking Information System (OTIS) prior to being allowed to participate in any activity or program. The results of all background searches are kept confidential.  Any applicant declining to complete a “Volunteer Background Check” acknowledgment form will not be permitted access past the building office throughout the district.   This form gives us your permission to perform a background check.  

This form will need to be filled out once per year, by each individual and applies district-wide for all children in the family.
