Level's Anti-Fraud Hotline is a reporting mechanism to anonymously report fraud, waste and/or abuse as outlined in Level's Anti-Fraud Policy.  Please fill out this form with as many details as possible. This can be used when:

 • You’re uncomfortable using Level's normal channels of communication (e.g. reporting the matter to a
    Supervisor or Compliance); OR
 • You’ve used other channels to raise concerns but have found them to be ineffective in resolving the issue; OR
