Transportation Flat Rate Enrollment Form - 2025-26
This form is the application for the Flat Rate Program for the 2025-26 school year.  Please note, by completing this form you are also agreeing to the student being removed from an assigned bus route.  Please read the program's guidelines and policies prior to submitting this form.

Please note, there is NO option for utilizing busing services in the AM, and picking up your child from school in the PM.  Also, choosing the Am-Only option does NOT place your child on a PM bus.  You will still need to reach out to your school and request PM busing.
Student Transportation ID *
Length of the ID is Seven or Eight digits (Cannot start with a 0).  The ID is used to uniquely identify the student and may differ from the student's school ID.  If you do not know the ID please reach out to your school. The ID is also provided on the communication sent to you via mail, stating that you may be eligible for the program.
Do you have a vehicle, and will be able to provide its Vehicle Information Number (VIN)?  Please note, the VIN is requested for payment each month, through the Monthly Payment Form. If you are unable to provide a VIN, a payment cannot be issued.  *
