1. Souhlas se zpracováním osobních údajů (včetně fotografických a video záznamů) podle čl. 6 odst. 1 písm. a) a čl. 7 Nařízení Evropského parlamentu a Rady (EU) 2016 / 679 z 27. dubna 2016 o ochraně fyzických osob při zpracovávání osobních údajů a o volném pohybu těchto údajů, kterým se zrušuje směrnice 95/46 / ES (obecné nařízení o ochraně údajů). Poskytnutím osobních údajů v rozsahu vyplněné registrace na aikido seminář dobrovolně uděluje Aikido Sokol Jihlava souhlas se zpracováním všech jím poskytnutých osobních údajů. Organizátor je oprávněn zpracovávat osobní údaje za účelem organizování tréninků, seminářů. Subjekt údajů má právo požadovat od provozovatele přístup k osobním údajům, které se jí týkají, právo na jejich opravu nebo vymazání nebo omezení zpracovávání, právo namítat proti zpracování, jakož i právo na přenositelnost údajů. Dotyčná osoba je oprávněna kdykoli svůj souhlas odvolat, aniž by to mělo vliv na zákonnost zpracování založeného na souhlasu uděleném před jeho odvoláním. V odůvodněném případě má dotyčná osoba právo podat stížnost dozorčímu orgánu, kterým je Úřad pro ochranu osobních údajů České republiky. Právo kdykoli odvolat souhlas, a to i před uplynutím doby, na kterou byl tento souhlas udělen, může dotyčná osoba uplatnit e-mailovou zprávou zaslanou na adresu vedouci.aikido.jihlava@gmail.com. Osobní údaje, jejichž poskytnutí je dobrovolné, budou uchovávány po dobu platnosti souhlasu a nebudou zpracovávány na žádný jiný účel, než na ten, na který byly získány. Osobní údaje se neprofilují, neposkytují třetím stranám (s výjimkou jejich poskytování příjemcům, kterými se v tomto případě rozumí externí organizátoři semináře - fyzické a právnické osoby - dojo / kluby, které se spolupodílejí na organizačním zabezpečení semináře) a nedochází k přenosu do třetí země. /// 1. AGREEMENT WITH THE PROCESSING OF PERSONAL DATA pursuant to Art. 6 ods. (1) (a) and 7 Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data (including photography and video) and on the free movement of such data, repealing Directive 95/46 / EC (General Data Protection Regulation). By providing personal data in the scope of the completed Aikido Registration seminar, the person concerned voluntarily grants Aikido Sokol Jihlava to the processing of all personal data provided by him. The organizer is authorized to process the personal data of the person concerned in order to organize training sessions, archiving of the achieved sports results, assignment of payment to the application for the duration of the membership of the person concerned in Aikido Sokol Jihlava. The person concerned has the right to require the operator to access personal data relating to him, the right to rectify or erase or restrict his processing, the right to object to the processing, and the right to data portability. The person concerned is entitled to withdraw his consent at any time without prejudice to the lawfulness of the processing based on the consent granted prior to his recall. In the justified case, the person concerned has the right to file a complaint with the supervisory authority, the Office for the Protection of Personal Data of the Slovak Republic. The right to withdraw consent at any time, even before the expiration of the period for which such consent was granted, may be exercised by the person concerned by e-mail sent to vedouci.aikido.jihlava@gmail.com. The provision of which is voluntary, shall be retained during the period of validity of the consent and shall not be processed for any purpose other than that for which they were obtained. Personal data are not processed, rendered to third parties (except for the provision to recipients, which in this case is understood as the external organizers of the seminar - physical and legal persons - dojo/ clubs participating in the organization of the seminar) and they are not transferred to a third country. *
2. Potvrzuji, že cvičím na vlastní odpovědnost a v případě úrazu nebo smrti si nebudu uplatňovat žádnou finanční ani nefinanční kompenzaci u organizátora. /// 2.
