FHS Band Instrument Rental Agreement 2019-2020
I understand that my student has elected to rent a school-owned instrument. The fee associated with this rental is $95 unless otherwise designated. This is a school board-approved fee. The length of term associated with this rental is either for the marching season (summer and fall), or school year as designated by the band director.

A specific instrument will be assigned to my student. I understand that I am financially responsible for the loss and/or damage of the assigned instrument (anything above normal wear and tear as determined by the band director). I also assume financial responsibility for the case, mouthpieces, and any accessories associated with this rental. I understand that I will be charged accordingly, even if someone besides my student causes the damage and/or loss. I understand that it is recommended that I take pictures and keep documentation to verify existing damage that exists on the instrument at the beginning of the rental period. My student will be given the opportunity to note all previous damage to the instrument on their check-out form which they will fill out when they receive their instrument.

I understand that my student has been provided a locker or designated space for his/her rented instrument and will store the instrument in that space while it is at the school.
