Community Service Hours
Hey APPHOmies! Please fill out this form if you are completing an APPHO event/pre-approved event for community service hours, or if you are submitting proof of hours from an outside source. Just as a reminder, the requirements for community service hours is as follows:

4 total hours for the semester
-2 hours are required to be through an APPHO (or pre-approved) community service event
-An additional 2 hours can be from an outside, health-related source such as volunteering or internships
