2018 - Summit Prep School Wide Camping Trip Driver Sign-up
Dates: Depart Thursday 08/23 (AM from Summit). Return Friday 08/24 (PM to Summit)

You will need to provide:
1) Proof of vehicle registration
2) Copy of valid driver's license
3) Signed driver form:

b. Spanish Version:
4) Chaperone Agreement:
5) Proof of Insurance (of vehicle you plan on driving on trip) with the following minimums:
$50,000 for personal injury to, or death of, one person,
$100,000 for personal injury to two or more persons in one accident
$50,000 for property damage.

-In addition to driving students - you may also be requested to take additional items to/from campsite.
-You are invited & encouraged to stay overnight in a permanent tent (provided), your own tent, or in your vehicle.
IF YOU ARE PLANNING TO STAY OVERNIGHT you will also need to provide:
1) Original or a high-quality copy of a negative TB test - no more than 60 days old from the first day of volunteering. Valid for 4 years. Instructions here:
2) Fingerprinting Clearance - only needs to be done once! Please print and use the SPS Request for Livescan:
This is the form that you must use to complete your Livescan. It contains a special code that will ensure your results are sent only to Summit. A list of LiveScan locations in Redwood City can be found here:
Allow at least two weeks for Livescan to process.
