Print the registration form at www.polodays.org OR fill out the document below.  Check-in will be Sunday, June 21st from 10:00am-12:15pm at the Polo High School Parking Lot (100 S. Union Avenue).  You will be given any and all information you need at that time. Please submit by Friday, June 19th.
