CHS Absence/Early Dismissal/Late Arrival Notification Form
PARENTS/GUARDIANS ONLY: Please complete this form to report an Absence, Early Dismissal request and/or a Late Arrival for your child. One form must be completed for each student. You do not need to email the school in addition to completing this form. The attendance office must receive this form from the parent's email on file in PowerSchool, or the absence cannot be excused.

All Early Dismissal requests must be submitted prior to 10:30 a.m.  If the form is received after 10:30 A.M., parents must pick up their child from school in person.This form may not be submitted by a student. Student self-submission will result in disciplinary consequences

***** Reminder: Early Dismissals for an appointment need to be followed up with a submitted medical note for an Absence to be excused, per CHS Attendance Policy. 
****  Notes must be submitted to the Attendance Office.
